Richland County Restaurant Inspections: Critical Violations March 18-23

The following restaurant inspections with critical violations were conducted by Richland Public Health between March 18 and March 23:

• Butler Elementary School, 125 College St., Butler, March 18. Responsible Person (PIC) unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). The ready-to-eat vegetables were in contact with the non-food contact surface of the carrots packed in the same bowl. Ready-to-eat raw cucumbers and broccoli were washed and placed in separate bowls from the carrots.

Foods poorly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Pre-packaged carrots with non-food contact surface wrappers were in the same bowl as ready-to-eat raw cucumbers and broccoli. The carrots were separated from the broccoli and cucumbers and placed in separate bowls.

• Madison Shell, 899 Ashland Road, Mansfield, March 18. Pressurized water is not available for all appliances as required (critical). Low level of hot water observed at the hand washing station. Correct as soon as possible.

• Large lots #5220, 929 N. Lexington Springmill Road, Ontario, March 18. Food packaging(s) received in poor condition (critical, corrected during inspection). Observed two cans of tomato soup and a can of pinto beans with a dent on the seal. PIC intentionally discarded products.

• Bell Stores #112, 1330 N. Lexington Springmill Road, Mansfield, March 18. Employees are not verifiably informed of their responsibility to report health information (critical, corrected during inspection). The employee could not find signed copies of the employee health policy. The employee had a blank copy for the employees to read and sign. Refrigerated, ready-to-eat, TCS foods not properly dated (critical, corrected upon inspection). Observed open packages of undated hot dogs, sausages and taquitos. PIC checked the date opened and the date correctly marked.

Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Eyed eye drops stored above drinks on the sales floor. PIC pulled to be relocated, separated from food items. Food display poorly protected from contamination by consumers (critical, corrected during inspection). Apples observed in customer self-service area not protected from contamination. Employee removed from sales area. Improper construction of equipment and/or utensils (critical, corrected upon inspection). Observed coffee beans stored directly in a non-food grade container. The employee voluntarily discarded the container and the product.

• The Golden Burrito #2, 1740 Park Avenue West, Ontario, March 21. Equipment food contact surfaces or utensils are dirty (critical, corrected upon inspection). Can opener observed to have food residue and buildup. PIC removed from the prep table and will purchase a new one. Correct by operation.

Insufficient air space between flood ledge and water supply inlet (critical, corrected upon inspection). The observed prep sink has no air gap between the spray head and the top of the sink basin. PIC able to adjust the spring on the spray head to allow sufficient air gap; consider removing the sprayer from the prep sink.

• Yana Mini Mart Inc., 98 Lexington Ave., Mansfield, March 21. Employees are not verifiably informed of their responsibility to report health information (critical, corrected during inspection). PIC unable to find signed copies of employee health policies. Provide the RPH Employee Illness Reporting Agreement Form; PIC signed and posted. Foods poorly protected from contamination by separation, packaging and segregation (critical repetition, corrected during inspection). Raw bacon seen on a shelf above canned drinks inside the cold room. PIC moved the bacon to the bottom shelf inside the cooler.

Foods poorly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cartons of raw shell eggs stored on shelf above ready-to-eat food inside accessible sandwich cooler. PIC moved the eggs to the bottom shelf. Improper storage of poisonous or toxic materials (critical, corrected upon inspection). Observed gallons of windshield washer fluid stored on a shelf above the canned drinks in the rear storage area. PIC moved the windshield washer fluid to the bottom shelf under the drinks. Missing and/or incomplete allergen declaration(s) (critical repetition, corrected during inspection).

Observed sleeves of Ritz and salty crackers for sale without a list of ingredients, common name, net weight or packing facility address on the package. PIC has withdrawn the crackers from sale. Food in an RFE must be embargoed due to adulteration, wrong brand, or expired baby food or infant formula (critical, corrected upon inspection). Observed two 12.6 ounce boxes of Nutramigen formula with an expiration date of February 1. PIC intentionally discarded the formula.

• The Richland School of Academic Arts, 1456 Park Avenue West, Mansfield, March 21. Mislabelled work containers containing poisonous or toxic materials. The sanitizer solution in the kitchen was not labeled with the common name (critical, fixed upon inspection).

• Aldi, Inc. #48, 955 N. Lexington Springmill Road, Ontario, March 22. Foods poorly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed raw ground turkey and pork sausage stored on ready-to-eat (RTE) wine.

• Mansfield Liberty Nursing Center, 535 Lexington Ave., Mansfield, March 22. CIP does not ensure that employees properly sanitize equipment and utensils and does not monitor sanitizer parameters (critical, corrected during inspection). The dishwashing machine observed does not have the appropriate sanitizing solution strength. PIC transitioned its employees to disinfection via manual dishwashing. Employees are not informed in a verifiable way of their responsibility to report information concerning their health (critical, corrected during inspection). The employee present at the time of the inspection did not have an Employee Health Agreement on file. PIC asked the employee to read and sign the agreement.

Chlorine sanitizer solution at incorrect temperature and/or concentration (critical, corrected upon inspection). The observed concentration of the dishwasher sanitizing solution was tested to be less than 10 ppm. The facility no longer uses the mechanical warewashing unit until it can be verified that the unit is capable of sanitizing with appropriate power. Equipment food contact surfaces or utensils not clean (critical repeat, corrected upon inspection). Visible black residue observed along the edges of the ice making area inside the bulk ice machine. The facility has stopped using it until proper cleaning and disinfection can take place.

Refrigerated, ready-to-eat, TCS foods not properly dated (critical, corrected upon inspection). Observed gallon of milk that had been opened for more than 24 hours and in cold storage without date. No air gap or approved backflow preventer on plumbing system (critical, corrected upon inspection). The observed bulk ice machine does not have an air gap at the edge of the drain. The facility has removed the bulk ice machine from service until an air vacuum can be provided.

• National promenade Duke/Duchess/BP1186, 1020 National Parkway, Mansfield, March 23. Work containers containing poisonous or toxic materials mislabeled (critical, corrected upon inspection). The spray bottle of disinfectant in the kitchen was not labeled with the common name.
